- 2% of the paid premium for individual plans

Become a broker for CareConnect
Start by calling us at 855-228-0541, or fill out our online forms.
Include a copy of your New York State Accident & Health License and Errors & Omissions Certificate with the completed broker agreement and broker application. You can submit these to us in one of two ways:
- Scan and email the completed forms to email@example.com, with the subject line “Application to become a broker.”
- Mail the completed forms to:

Emailing your application is the fastest way to become a broker for CareConnect.
Once we receive completed materials, it will take 3-5 business days to become appointed.
Note that you must be appointed at the time of sale to receive commissions.
